We welcome patients of all ages. We are glad that you have decided to visit Rebound Physical Therapy. Below is a list of forms that you can print out and complete prior to your first visit.
Rebound Physical Therapy is a preferred provider for Blue Cross, Blue Shield, United Health Care, Aetna, Cigna PPO/HMOs, Medicare, Tricare, and most Worker's Compensation plans. If you have any questions about your insurance, please give us a call.
Remember to wear loose and comfortable clothing. If you are receiving lower extremity treatments, we encourage shorts. For a shoulder problem, a tank top is a good choice, and for low back problems, wear a loose-fitting shirt and pants. You are welcome to bring a change of clothing.
